Teacher Training Public Classes

Assumption of Risk

I understand that yoga, movement practices, breathwork, and related wellness activities involve physical exertion and may include stretching, strengthening, balance, and hands-on adjustments. I acknowledge that participation carries inherent risks, including but not limited to muscle strain, joint injury, dizziness, fainting, or more serious injury, and that these risks cannot be entirely eliminated.

I voluntarily choose to participate with full knowledge of these risks and assume full responsibility for my participation.

Medical Acknowledgment

I understand that yoga and related practices are not a substitute for medical care, diagnosis, or treatment. I affirm that I am physically able to participate and that I have consulted a licensed physician regarding any medical condition that may affect my ability to safely participate.

If I am pregnant, post-natal, post-surgical, or managing a medical condition, I confirm that I have received my healthcare provider’s approval to participate.

Health & Wellness Agreement

I agree not to attend or participate in any class or event if I am feeling unwell or experiencing symptoms of illness, including but not limited to fever, cough, sore throat, shortness of breath, nausea, vomiting, diarrhea, or any contagious condition.

Substance-Free Participation

I affirm that I will not participate in any class, session, or event under the influence of alcohol, drugs, or any substance that may impair my judgment, coordination, or ability to safely engage in physical activity.

Release of Liability

In consideration of being permitted to participate, I hereby irrevocably release, waive, discharge, and hold harmless:

• The Yoga Tree LLC (Haverhill, MA)

• Light House Yoga (North Andover, MA)

• The Academy of Yoga Teacher Training Program

• All owners, directors, employees, instructors, teacher trainees, contractors, volunteers, agents, and affiliates

from any and all claims, demands, causes of action, or liability of any kind arising out of or related to my participation, including but not limited to claims of negligence, to the fullest extent permitted by law.

I understand and agree that participation is entirely at my own risk.

Personal Property

I acknowledge that I am solely responsible for my personal belongings and that the released parties are not responsible for loss, theft, or damage of personal property.

Code of Conduct

I agree to conduct myself in a respectful and safe manner. Threatening, abusive, harassing, obscene, or illegal behavior will not be tolerated and may result in immediate removal from the program or class without refund.

Photo & Video Release

I understand that photographs and/or video recordings may be taken during classes, trainings, or events for educational and training purposes, including teacher feedback, assessment, and documentation.

Governing Law

This Waiver and Release shall be governed by and interpreted in accordance with the laws of the Commonwealth of Massachusetts.

Acknowledgment & Acceptance

By accepting this agreement or by participating in any class (in person or online), I confirm that:

• I have read and fully understand this Waiver and Release

• I am signing it voluntarily

• I understand that this is a legally binding agreement and a complete release of liability

I agree to these terms as a condition of participation.
